Terms and Conditions – Please read all:

Psychotherapy Prohibition: The California laws and ethics of psychotherapy mandate boundaries between the healing and somatic arts (massage), and psychotherapy. Under those rules a massage client is a massage client and a psychotherapy client is a psychotherapy client. Therefore, no current psychotherapy client of mine may receive massage from me.

24 hour notice for any appointment:  As a courtesy to all, please give twenty-four hour notice if you need to change your appointment. Cancellations with less than twenty-four hour notice are charged at 50% of the appointment fee.  No-show appointments are charged 100% of the appointment fee.  Thank you in advance for your consideration. 

Psychotherapy Clients: The California laws and ethics require you to be informed of rates before service, require you to give informed consent for treatment, understand that treatment varies based upon the individual, understand that HIPAA governs information shared with insurance agencies, understand that you hold privilege to your records and how the courts may require them, understand the laws concerning minors and their consent, secrets therapy policy, and that psychotherapists are mandated reporters for child abuse, dependent abuse, elder abuse, threats to self, threats to others, and major property damage.

Psychotherapy rates – as of January 1, 2018 are:  Intake is 100 min and rate is $300. Session rate is 50 min and rate is $150. I do not take insurance, but if requested will provide documentation of service so that client may process the claim with their insurance carrier.

Psychotherapy paperwork: The California laws and ethics, as well as insurance laws, related to psychotherapy require you to be fully informed in writing and to fill out numerous forms for treatment. Those forms will be given to you before or at the beginning of treatment. No one may begin treatment before the forms are completed.

Healing and Somatic Arts Clients (massage):

Please read the above Psychotherapy Prohibition.  Similarly, no psychotherapy will be given during a massage session.

Deep body work may be intense and as such your breath is your friend. Long slow deep breathing is very helpful. The therapist will check-in with you, but always speak up if something concerns you. Please drink a lot of water after the session as your body cells have been squeezed out. It is suggested that you do not workout for 24 hours after the work.

Healing and Somatic Arts rates – as of January 1, 2018 are:

• 50 min $120
• 80 min $160
• 110 min $ 190

Please note:  Credit cards incur a processing fee of 3%. Travel to your local location will incur an additional fee that varies between $25 and $50. Travel beyond local to be paid for by client.

Massage waiver and release of liability:  The massage therapist must be made aware of existing physical, and medical conditions, and kept informed of any changes to physical, and medical conditions.  The Waiver and Release of Liability must be filled out fully, and signed to receive healing and somatic work.  Parents must sign for their minor children.

Medical insurance for massage:  May possibly be accepted with a physician’s referral in the La Jolla Shores office.  The receptionist there will help your inquiry. 

No discrimination:  Jacob does not discriminate on the basis of race, creed, color, ethnicity, national origin, religion, sex, sexual orientation, gender expression, age, height, weight, physical or mental ability, veteran status, military obligations, and marital status.
